In order to change the amount of income tax withheld from your pension, you must complete new federal and provincial income tax forms (TD1s) available on the Canada Revenue Agency’s website and submit them to SHEPP. If you reside in a province other than Saskatchewan, you must complete the appropriate provincial TD1 form for your province of residence. For more information refer to the Tax Forms Information Sheet.
Authorization for Third Party Access to Private Information
Complete and submit this form to authorize SHEPP to share your personal information with the third party you identify. It is SHEPP’s policy not to disclose a member’s personal information to a third party unless the member has provided SHEPP with written authorization to do so.
Direct Deposit Authorization
Complete and submit this form to ensure your monthly pension payment is deposited directly into your account.
